Please select the exams you wish to take: If there is a scheduling conflict (two exams offered at the same time, approved Late Testing conflict) please contact Mr. McLean immediately and state the conflict in the "Additional Information" box at the bottom of the form.

Please check with your AP teacher to confirm the name of the correct exam to take.
Special Accomodations
Students with documented disabilities may be eligible for accommodations on AP Exams. However, in order to take an AP Exam with accommodations, a student must be approved for accommodations by the College Board’s Services for Students with Disabilities (SSD) office. Having an IEP or 504 Plan does not automatically constitute authorization for testing accommodations; students must be approved by the College Board. If you believe you are eligible for testing accommodations, please see your counselor immediately to request that your SSD accommodations report be sent to Mr. McLean prior to March 1st. Some examples of accommodations include large-type exams, large-block answer sheets, and extended time on tests.
Travel to Offsite AP Exams
Some Advanced Placement exams will be off campus due to classroom availability and space. (For times and locations, please consult the schedule of exams when it is posted in April – typical locations are the University of Portland and the Archdiocesan Chancellery Office). Since Central Catholic does not provide transportation to these exams, we ask that you complete the information below.

Remember: Students should plan to arrive a half an hour prior to scheduled exam times. Please give yourself plenty of time to account for traffic, parking, and navigating an unfamiliar college campus. Students will be dismissed from morning exams no later than 12:00 p.m. Afternoon exams usually end no later than 4:00 p.m. Students who do not have other AP Exams scheduled in the afternoon will not be required to return to the school campus. The reverse is also true for students taking afternoon exams: they do not have to come to school on the morning of the afternoon exam.
Notification of Approved Absence from School
In lieu of the paper form (“Orange Form”) signed by your teachers, counselor, and dean, indicating that you will miss class-time to take an AP Exam, the names of every examinee at each exam will be sent out to all teachers and administrators prior to the exams. Attendance will be taken at all AP exams. Students who register for an exam but decide not to take the exam are then expected to attend their normal classes in school on that day.

I will meet with my teachers before the exam to work out what I may miss. I am responsible for all work missed, including tests. Homework and tests should be completed prior to the excused absence unless other arrangements have been made with the teacher. The assignments must be completed and turned in immediately upon returning to school. All tests will be taken on the day of return.

Fees
Fee reduction is available to students with financial need. Please see Mr. McLean for details. Please note: for the Fee-Reduced exam, you must qualify for the reduced-price lunch program through the PSAS process. If you are unsure, choose the reduced exam fee as all student names will be submitted to Mrs. Martin for verification.
Payment and Digital Signature
Your digital signature and submission of this form indicate your commitment to take your selected exam(s). You will be billed automatically for all exams you have committed to take. If you need to modify your submission, please contact Chris McLean at cmclean@centralcatholichigh.org.
After this form is submitted (by clicking “NEXT” below and working through the remaining two “Payment” and “Review” forms,) you will receive an email to the address submitted above and a receipt in school mail that confirms your exam registration. If you do not receive an email and paper receipt within one week of registering, then we may not have your registration. Please email cmclean@centralcatholichigh.org for assistance.
Clicking "Next" (below) will take you to the payment page. Unless you choose to pay online at this time, you may choose "Bill Me Later" (under “Payment Method” on the next screen) to have the final charge for exams appear on your May 2018 Student Billing Statement. There are no refunds once the exams are ordered. We, the undersigned, understand that this registration form will be used to determine the number of tests to be ordered by Central Catholic High School, and intend to take the exam(s) we have selected.
